Comprehensive Housing Support

At Open Arms Community Health & Service Center (OACH & SC), we are dedicated to offering top-notch housing assistance to adults, families, transitional-aged youth, and individuals re-entering the community from incarceration who face homelessness or mental health challenges. Our housing program provides immediate access to:

Our Housing Coordination Team advocates for participants to ensure they secure safe, affordable housing. We also offer housing sustainability services to help prevent eviction and connect participants to additional housing programs and benefits through local and county collaborations.

Comprehensive Housing Support

We help individuals and families who are homeless or at risk of homelessness navigate a path to stable housing. Participants are assigned a Lead Care Manager who assists with securing permanent housing, managing deposits (including security and utility deposits), and accessing temporary and permanent housing solutions, supported housing programs, and shelters. We also facilitate transitions from hospitals, post-hospitalization, LTSS care facilities, nursing homes, and incarceration.

We link individuals to programs that provide housing deposit assistance, helping to identify, coordinate, and secure one-time funding for moving needs, ensuring a smooth transition to stable housing.
